IPH Receives $150,000 Grant from the Mother Cabrini Health Foundation to Expand Community Connections Program

IPH is proud to announce it has been awarded a $150,000 grant from the Mother Cabrini Health Foundation. This funding will directly support IPH’s Community Connections program, a low-barrier day-time drop-in resource center designed to meet the immediate needs of people experiencing homelessness or at-risk of becoming unhoused.


Community Connections offers essential resources including food and meals, clothing, laundry, showers, lockers, and mailboxes. It also provides personalized support for individuals to connect with mental health services and address substance use issues.


With this funding, IPH will expand its capacity to serve Albany’s low-income residents by providing additional case management and harm reduction services. Having two instead of one Case Manager will allow more opportunity to work one-on-one with guests to address their unique needs including employment, housing, and health-related social services. A Harm Reduction Specialist will provide targeted support for people who use drugs, offering educational workshops, Naloxone training, and access to harm reduction tools like fentanyl test strips.
